Dear Parent/Guardian,

E-consent for your child’s HPV (Human Papillomavirus) vaccinations

We are writing to advise you that the School Aged Immunisation Team will soon be visiting your child’s school to deliver the first dose of their HPV vaccination course.

To get the best protection, your child will need two injections with the first being given in the summer term of 2021 and the second in the spring/summer of 2022. Please see communication from your school for exact dates.

The HPV vaccine is now being offered to all students, male and female and offers protection against infection from the Human Papillomavirus (HPV) which can lead to a range of cancers in later life. It is being offered free to your child in school. (Please note this vaccine is not routinely available from your GP).

Please submit your form prior to the deadline to ensure your child receives their vaccination in school. The link will close 7 days before the planned vaccination session. Please note that once the deadline is passed you will only be able to access the vaccination from a community catch up clinic.

Please follow the link below to the 0-19 website to complete the consent form for your child. If you do not want your child to receive the vaccinations, please complete and return the form declining your consent. Please be assured that if we receive a refusal of consent, we will not vaccinate your child.
