Year 8 & Year 9 E-Consenting for Teenage Booster Vaccinations 2021

Dear Parent/Carer

E- Consenting for your child’s teenage booster vaccinations.

Td/IPV (Tetanus, Diphtheria,Polio) and Men ACWY (Meningitis ACWY)

We are writing to inform you that the School Aged Immunisation will Team visiting your child’s school soon to deliver the Teenage Booster vaccination programme. Please see communication from your school for exact dates.

Please note: Due to the closure of schools during lockdown we have been offering these vaccinations at our community clinics. If your child has already attended a clinic for these vaccinations you will not need to complete the e consent form.

Due to the closure of schools last summer we were unable to complete the booster dose of your child’s HPV vaccination. If you have given consent for this vaccine we will be able to administer this outstanding dose when we visit for the teenage boosters.

Teenage booster vaccination | NHFT

Please ensure you submit your form prior to the deadline to ensure your child receives their vaccination in school.  The form will close 7 days before the planned vaccination session. Please note that once the deadline closes you will only be able to access the vaccination from a community catch up clinic. 

If you do not want your child to receive the vaccinations please complete and return the form declining consent. Please be assured that if we receive a refusal of consent we will not vaccinate your child.  Please be aware that if we do not receive a completed consent form before the vaccination session we may offer your child the opportunity to self-consent on the day. This is in line with the ‘Gillick’ Competency Framework.

While COVID-19 is in circulation, the healthcare team and school will follow guidelines to ensure children are safe when being offered the vaccine.

If your child is absent on the day of planned vaccinations, you will be offered the opportunity to bring them to a community based vaccination clinic.
